You have a direct relationship with our Providers, no middle man (insurance) involved.

We Do Accept Patients With or Without Insurance. We do not bill your insurance.

We Charge A Low, Affordable Monthly Fee for the personalized care we provide. Instead of charging every time you visit the office. There are no additional fees or copays for coming to our office to be seen. Why do we do this? There are many advantages of doing things this way:

We are more than willing to handle matters over the phone or through secure messaging since our revenue does not depend on a busy office.

Our office is less busy, so when you need to have a visit, there is usually very little wait.

You are not penalized for being sick by having to pay more out of pocket.

You never have to worry about having enough money to visit our office.

Membership/Services

All Monthly Membership Services Include:
Unlimited Physician Visits, In-Office Procedures such as minor laceration repair (see other Service), In-Office Diagnostic Tests (see other Services- strep test), Discounts on other services we offer at our location (Aesthetics, Nutritional IV Therapy, Weight Loss, etc.), Discounted X-rays, Mammograms, Prescriptions and Labs at one of third party locations. 24/7 Telephone/Text Access, 24/7 Email Access, online access to your medical records at any time through our secure patient portal.
The Membership or Service(s) you choose and/or First months membership fee(s) are collected from you by either Direct Debit, Cash or Credit Card upon your initial visit and are non refundable under any circumstance once services are provided.

There are no extra fees for upgrading your membership.

Your Direct Debit or Credit payment for the monthly membership fee(s) will automatically be charged according to our Financial Policy Agreement reviewed and signed by you.

If any Direct Debit, Check or Credit card payment is returned unpaid we may charge you an admin or return fee up to $20.

If payment is not complete as per the membership agreement, you acknowledge the membership services will terminate immediately.

You agree to advise us immediately on any change to the Members Details provided to us on file and/or payment method updates.
